Our financial personnel are experienced well trained staff and are very familiar with insurance and payment matters. They stand ready to assist you with questions you may have regarding your coverage and your insurance policy.

Our fees are based on what is termed reasonable and customary charges for the specialty and geographic location. Our office visit charges are based on several factors which include complexity and time spent handling the problem.

Surgical procedures and care fractures often fall under the term “global fee”. The global fee entitles the patient for care of the problem and a period of follow up included under the same payment.

Payment for office visits and services are expected at the time the service is rendered. Many insurance policies have deductibles and copays. You may pay for the services with cash, check or credit card. We accept Visa and Mastercard.

Unfortunately the administrative costs continue to rise and therefore we retain the right to levy an administrative charge of $20 to cover the cost of billing and collecting copays and deductibles if not met at the time of your visit.

If you have an outstanding balance we appreciate payment in full. If you have difficulty meeting outstanding balances our collection staff is instructed to work with you to arrange a payment plan.

In the event that we make several attempts to collect outstanding balances and are unsuccessful, we reserve the right to turn the account to a collection agency. If an account is turned to collections, you will be responsible for interest accrued, legal and collection fees.

Returned checks will incur a $35 fee per check.


Medical LogoSuburban Orthopaedics participates with most insurance carriers and we are more than happy to submit your claim to the insurance company on your behalf. The more accurate the information you provide us the more efficiently we can complete this process. We ask you to please provide us with any referral forms required by your insurance company. We also ask that you keep us informed of any address, phone number changes or changes in your insurance policy.

Worker’s Compensation

If your injury is work related and you are eligible for workers compensation benefits, workers compensation must be authorized and need to provide us with the following information:
  • Claim number
  • State where injury occurred
  • Date of injury
  • Necessary claim forms
  • Names, addresses and phone numbers of
  • Your employer
  • Your supervisor
  • Your W/C carrier
  • W/C adjuster
  • Attorney's name and phone number if one has been retained
